Compliance and Transition Program

In this program we use the art of moving you from knowing what to do, but not knowing how  to do it, to the point where you not only find a way, but also find reasons to. In doing so, you discover where you are now, develop a healthy vision of yourself, examine obstacles that may hinder your progress, and through support and careful planning, develop tools to over come them.

This type of coaching is unique because we understand compliance and transition is motivated not out of fear or outside pressure, but, based upon your personality and experiences. We use various specialized assessments to help engage thought provoking ideas depending on your need. Therefore, each session is tailored fit just for you. And, although we guide you through the gap from where you are (non-compliant or transitioning) to where you desire to be (transitioned and healthier), it is your intrinsic motivation, faith, and commitment that drives the change. Doing this will not only build your confidence, but it will help you see beyond your illness in a way to help impact others around you.

Why You Want It

You have moved into a new phase of your life. Whether you have just found out about your chronic illness, your need of a transplant, or your compliance is breaking down, it all involves some type of transition and transitions can be scary. Fear often keeps us confined to a box of familiarity. When we do that, we live like how we used to do. Living like we did in the past is not going to work for us in our present or future. With our Compliance and Transition program, you learn the tools you need to move forward and be inspired to be a healthier you. You will ask questions of yourself and find your reason to stay compliant. Steadily you will find your own path in doing so. Soon you will come to the realization that your sickness can not hold you back – if you take control of it. All of that can come out of really awesome coaching (which we just so happen to provide *smile*)

Patient Support and Health Literacy Program - Limited Access, please call!

With this program, you have access to a dedicated volunteer whose only responsibility is to be available to you when you need further help understanding your health care instructions. This person will help you:

  • Learn how to talk with, ask questions to, and listen to your doctor to get the answers you need during your appointment
  • Take notes during your appointments and surgeries if you have no support available
  • Understand your doctor’s orders, medication directions, and health forms
  • Build needed support, coordinate at-home care, and research resources to be sure you have help for follow-up care.

Why You Want It

We’ve heard from patients, and parents of pediatric patients alike (especially those newly diagnosed), about how overwhelming appointments and post surgeries are. They have so many questions to ask, instructions to keep up with, and information to process. They’ve admitted, in the “fog” of it all, many important details can get lost. We also know that without a support person already in place, it will be harder to find someone to fill that critical role that a patient needs to heal and move forward. Chances are, that you’ve been there, and know the importance of a program like this. Or, you are there now and need a program like this. If so, contact us.  Get connected with one of our volunteers who will be your personal liaison – assisting you in asking questions, gathering information, and taking notes.  For those times that you are “in the fog”,  just can’t think, don’t have the support, or are in a situation where the information given is hard to understand, be confident in knowing that you have competent help. We will be with you listening and taking notes on important details that you may miss. Afterward, meet with your volunteer (when you have a clearer head) to go through your information slowly so that you can understand your instructions. It’s easy and just another service that we provide to keep you moving forward!

Group and One on One Services

Each service comes in both group coaching (for those clients who find motivation in the company and interaction of others) and one-on-one coaching (for those who prefer the individualized and personal attention of the staff). Since every situation is different, there are opportunities to go between both group and one-on-one as needed.
